IUSD Health Services would like to remind our families of some important items that need to be completed at the beginning of the school year.
- Complete “Data Confirmation” in the Parent Portal to ensure that we have the most up-to-date information about your student(s), which includes their health information. This information is utilized by your student’s school nurse to ensure that your student receives the necessary care while at school. For students with extensive medical needs, please reach out to your school nurse within the first week of school.

- California state law requires all students to be up-to-date on all school-required immunizations before attending class in person. Please ensure your student is current on all required vaccines before the first day of school.
- Students who require medications at school must have a Medication Authorization form completed by their provider. Once completed, this form can be dropped off at the school health office along with the medication. Please ensure all medication is in the original packaging. If the medication is a prescription, please ensure the prescription label is attached.
