CHILD HEALTH AND DISABILITY PROGRAM

BOARD POLICY - 5141.3

The superintendent or designee shall verify that students have complied with legal requirements for a comprehensive health screening and immunizations at school entry. In order to assure optimum health of students and comply with Child Health and Disability Program (CHDP) state statute, the Board requires all students to have a health assessment as outlined below:

First entry into school

Physical Exam

TK

No more than 12 months prior to entering Transitional Kindergarten.

This exam will also qualify for Kindergarten.

** TK students will need to submit a new physical exam prior to 1st grade.

Kindergarten

No more than 12 months prior to entering Kindergarten.

**If the physical exam is less than 6 months prior to Kindergarten, an additional physical exam will NOT be required for 1st grade.

1st grade

No more than 18 months prior to entering 1st grade.

In order to enter the schools of this District, all transitional kindergarten, kindergarten and first grade first time entrants must provide a certificate approved by the California State Department of Health or a qualified physician documenting that the child has received the appropriate health assessment.

Under California Law, a CHDP Health Assessment waiver (PM171B) may be signed by the parents or legal guardians indicating that they do not want or are unable to obtain a health assessment. This form, available at the student’s school, shall be accepted by the school in lieu of the Report of Health Examination for School Entry certificate. This waiver applies to the health assessment only and does not include immunizations. (See Board Policy 5141.31 Immunizations.)
