ADMINISTRATIVE REGULATION - 4164.2
Leave may be granted a permanent employee requesting absence for medical or maternity reasons. Such absence will be in paid status, if eligible, under state or federal law.
- Submit Application for Leave of Absence to Human Resources for approval.
- Attach a disability verification form signed by a medical doctor or recognized practitioner completed in detail stating reasons for the requested leave and specifying beginning and ending dates of the leave.
- Leave may be granted for a period of up to one year.
