
What information and materials must I bring with me? (once I have completed online enrollment)
Verification of Identity of parent, licensed foster parent, or California Superior Court-appointed legal guardian
- A Drivers License (any photo drivers license is permitted) or passport with photo ID
Verification of residency inside IUSD of parent, licensed foster parent, or California Superior Court-appointed legal guardian. For initial enrollment, two forms of residence verification are required from those listed below:
- California Vehicle Registration with a current address. (Must be an official DMV document.)
- California Drivers License with a current address. (Must be an official DMV document.)
- Electric bill (both parts, in English) or verification of electrical service connection.*
- Southern California Gas bill (both parts, in English) or verification of gas service connection.*
- In the case of a home purchase, a Closing Escrow Agreement. (Note: The district does not accept opening escrow papers or lease agreements as verification of residency.)
- Current Property Tax or Income Tax Documents
- Current Bank Statements/Checkbook with current address
- Current Cable Bill
- Current Water/Trash Bill
- Current Payroll Stub
- Current Telephone Bill (landline, not cellular)
- Current Social Services documents
- In the case of UCI Housing, an official UCI document asserting the person resides in student housing, which usually covers gas and electric.
- In the case of purchasing a home under construction, a New Communities K-12 Enrollment Address Verification form may be obtained at the sales office and will be accepted as temporary residency verification.
* Note: In the event a utility service connection is used as proof of residency, then a utility bill (both parts, in English) must be provided within 45 days to assure continued enrollment.
Proof of Age is required:
- Official or Certified Copy of Birth Certificate, or
- Current Passport, or
- Immigration Certificate
- Baptismal Certificate
Proof of Immunization Records are required for certifying up-to-date immunizations for each student. All Kindergarten students must have completed a physical examination no more than six months prior to entering Kindergarten. (If an exam is completed between 6-12 months prior to Kindergarten entrance, another physical exam will be required prior to entering first grade.) All First Grade students must have completed a physical examination no more than 18 months prior to first grade entry.
Emergency telephone numbers are required, including an out-of-state emergency number.
Helpful Documents and Information
An academic transcript or report card from your former school. (Prior school records will be requested by the school in which your child enrolls.)
For Special Education students: Prior IEP and Assessments
For GATE (gifted and talented) students: Prior test scores and/or certification forms
